Honestly just about every doctor I've ever seen was originally from India or was Asian. If Canada or the US were to charge a fee for permission to remain the revenue would be astounding! I can't think of one immigrant that would refuse to pay a $500 monthly fee to remain and work freely in either country. It has been suggested many, many times. The amount of revenue generated would exceed that of the tax revenue we currently have in place for common Americans by 2030. Why Republicans don't allow this?
